Physical Therapist - PRN

Summary: Perform patient evaluations and plan, implement and adjust individualized treatment plans. Work collaboratively with other healthcare professionals to ensure patients reach maximum level of function.

Duties and Responsibilities:

• Provide direct patient care, evaluate and reassess functional needs and consult with other specialist as needed and adjust care plan as indicated.
• Conduct therapy practice within defined standards of care as set by Illinois Physical Therapy Practice Act and department procedure.
• Delegate and supervise specific treatment to PTA and support staff in order to achieve patient's functional goals.
• Document evaluations, treatment goals/plans and regularly update documentation. Maintain all required prescriptions and signatures.

Education: Graduate of accredited Physical Therapist program and possess Illinois license (or temporary work permit).

Experience: One year preferred in hospital setting.
