RN/LPN Case Manager - PRN

Summary:

The Case Manager works within the hospital, and assumes clinical management and outcomes of high cost/high risk patients by working with health care providers, patients, and families. Performs hospital stay precertification. Assists with comprehensive care planning, implementation of care plans, and communication/evaluation of progress towards goals. Ensures appropriate level of patient/family health knowledge and understanding when developing the discharge plan.

Responsibilities:

1. Maintains an awareness of an ongoing compliance with applicable laws, regulations and policies and procedures in area of responsibility.
2. Improves patient satisfaction. Uses obtained data to identify performance improvement opportunities for case management service area. Reports to CNO any complaints received from patients or their representatives for prompt service recovery and follow up.
3. Improves physician satisfaction by identifying and reporting needs of the physician that will enhance efficiency of the service area.
4. Develops and implements a patient discharge plan in coordination with the patient/family/caregiver and with their preferences in mind. Performs on-going assessment for any discharge needs and follow- up appointments.
5. Reviews all patient stays for appropriate admission criteria and levels of care. Keeps physicians informed regarding review results.

Qualifications: BLS certification required. Must hold RN IL License. In-Patient Acute Care setting experience is required. Case Management experience preferred.
