Summary: Perform patient evaluations and plan, implement and adjust individualized treatment plans.     Work collaboratively with other healthcare professionals to ensure patients reach maximum level of function.

Reports to: Rehab Manager

Titles Supervised: NA

Essential Duties & Responsibilities

  1. Provide direct patient care, evaluate and reassess functional needs and consult with other specialist as needed and adjust care plan as indicated.
  1. Conduct therapy practice within defined standards of care as set by Illinois Occupational Therapy Practice Act and department procedure.
  1. Delegate and supervise specific treatment to COTA and support staff in order to achieve patient's functional goals.
  1. Document evaluations, treatment goals/plans and regularly update documentation. Maintain all required prescriptions and signatures.

  1. Comply with all hospital policies and procedures as applicable for position including employee handbook, department and administrative policies and procedures, goals and objectives for self, department and performance improvement, safety compliance program, HIPAA and confidentiality, accreditation, and survey standards and infection control.
  1. Provide exceptional customer service to all external (patients, families, etc.) and internal (co-workers, other departments, physicians, etc.) customers.
  1. Participate in staff meetings and serve on hospital committees/teams as appropriate.
  1. Perform work in effective, organized, accountable and competent manner including following oral and written instructions.
  1. Be respectful, courteous and caring in all interactions with others.
  1. Demonstrate initiative and ownership in performance of duties and be self-starter.
  1. Pass competency and proficiency testing as required by department or hospital including computer based testing.
  1. Adapt to changing work assignments and comfortable serving diverse community
  1. Promote hospital services in service delivery area.
  1. Function well under stressful conditions involving other departments, workload, etc.
  1. Keep knowledge and skills current via education activities, in-services and other training.
  1. Report to next shift or manager any problems or areas of concern to correct.
  1. Perform related duties as needed including assisting others with their work.
