Position Description

Clinical Manager Massac Medical Clinic - FT

Summary:

Responsible for managing the daily operations of the clinic including personnel management and oversight of patient flow.

 

Reporting Relationship:

The Clinical Manager Massac Medical Clinic reports to the CEO and directly supervises the following positions:

  • Medical Assistants
  • Medical Office Supervisor

Qualifications Required:

Registered Nurse with a license to practice in the State of Illinois and a minimum of five years in a healthcare setting preferred, Licensed Practical Nurse with a license to practice in the State of Illinois and a minimum of five years in a healthcare setting and a Bachelor's Degree in a healthcare related field; excellent verbal and written communication skills; ability to work independently and as a team member; strong interpersonal, listening and critical thinking abilities; excellent organizational skills 

Essential Job Duties:

  1. Clinical Operations: Manage workflow to ensure timely and high quality task completion and effective distribution of work among support staff. Monitor clinic processes to ensure effective and efficient operations; monitor efficient patient flow patterns to maximize clinical and staff efficiency; develop and implement procedures for improved utilization and operations. 
  2. People Supervision: Maintains a routine presence during hours worked by assigned team members. Ensures that new team members are appropriately oriented, trained and mentored to facilitate their effectiveness in the role they are performing. Improves team member satisfaction and engagement in areas of responsibility. 
  3. Process Improvement: Identify opportunities for improving the cost and/or quality of care provided in the clinic. Develop recommendations for improvement and implement new processes with the support of the Director. 
  4. Expense Management: Assists Director in preparation of operating budgets to reflect an increase in value per patient served. Manages expenses to reflect actual volume utilizing budgeted levels as the base. Considers overall labor hours, premium labor hours, supplies, purchased services and professional fees when making ongoing adjustments to budgeted levels in areas of responsibility. 
  5. Payroll: Prepares and approved bi-weekly payroll for all members of the Medical Clinic, including providers and support staff. 
  6. Compliance Management: Maintains an awareness of and ongoing compliance with applicable laws, regulations, or policies in areas of responsibility. 
  7. Care Management
    1. Care Coordination
      1. Pre-Visit Planning: development of pre-visit plans with complete patient information prior to a scheduled follow-up visit for the care managed condition.
      2. Care Planning: in partnership with the patient and the care team design, implement and monitor individual patient care plans. Ensure that treatment goals are assessed at each visit and assist the patient in addressing barriers to progress.
  • Medication Management: Conduct medication review and reconciliation at planned care management visits (either in person or over the phone), care transitions, and following visits to specialists, emergency departments or hospitalization. Ensure documentation of all prescription medications and over-the-counter medications, herbal therapies and supplements.
  1. Referral Management: Ensure that each care managed patient has successfully made an appointment and was seen by specialists as referred by their primary care provider. Ensure visit information is received from each specialty visit and incorporated into the patient's electronic medical record and is available for review by the patients primary care provider.
  2. Self-Management Support: provide care managed patients with a written self-care plan that addresses the patient's condition, and includes goals and an action plan for monitoring self-care. Refer patients to community resources for education and support as needed.
  1. Patient Centered Medical Home (PCMH)
    1. Compliance Monitoring: Ensure that the designated primary care practices are compliant with all requirements for achieving and maintaining PCMH status.
    2. Data Reporting: Collect and report required data for PCMH deeming body in a timely and complete manner.
  2. Accountable Care Organization (ACO)
    1. Commitment Monitoring: Ensure that all of the quality of care commitments made to the ACO are delivered by documenting and tracking each commitment and partnering with the Director of Professional Services to ensure each is carried out.
    2. Data Reporting: Collect and report all required data to the ACO in a timely and complete manner.
  3. Quality Improvement
    1. Develop a quality improvement dashboard that tracks all of the quality metrics for the PCMH and ACO.
    2. Collect and report on each quality improvement measure in a timely and complete manner. 

Shift:

Monday - Friday   8:00 am to 4:30 pm
